Etidronate disodium CAS No. 7414-83-7
Category: Drugs and drug intermediates
Product Description
Etidronate disodium is a first-generation bisphosphonate that slows down excessive bone turnover by binding to hydroxyapatite crystals and inhibiting osteoclast activity, thereby reducing bone resorption. The drug is primarily used to treat Paget's disease of bone, heterotopic ossification (e.g. after hip surgery or spinal cord injury), and hypercalcemia caused by malignant tumors; it can also be used to treat postmenopausal or male osteoporosis when newer drugs are not applicable. The usual oral dose is 20 mg per day, taken with a glass of water on an empty stomach, and then taken upright for at least 30 minutes; the dose will be adjusted according to other indications. Common adverse reactions include gastrointestinal discomfort (nausea, diarrhea), headache, and musculoskeletal pain. Rare but serious risks of long-term treatment include osteonecrosis of the jaw and atypical femoral fractures. Calcium, iron, and antacids must be taken separately to avoid interference with absorption, and patients with renal insufficiency or esophageal disease should take it with caution. Note: We supply etidronate disodium raw materials, not finished drugs.

Paget’s disease of bone
Post-menopausal & male osteoporosis (cyclical regimens)
Heterotopic ossification (after hip surgery, spinal cord injury)
Hypercalcaemia of malignancy
Bone metastases & multiple-myeloma bone disease
Calcific uraemic arteriolopathy (calciphylaxis) – case-report success
Generalised arterial calcification of infancy (GACI) – off-label paediatric use
-
Dosage & Administration
Standard oral: 200–400 mg daily on empty stomach, or 200 mg day⁻¹ × 14 days every 12 weeks (cyclical)
Paediatric GACI: ≈ 18 mg kg⁻¹ day⁻¹ until arterial calcification regresses
Intravenous infusion for severe hypercalcaemia
-
Adverse Effects & Precautions
Common: GI upset (nausea, diarrhoea), headache, bone pain
Rare/Serious: osteonecrosis of jaw, atypical subtrochanteric fractures, hypophosphataemic rickets with long-term high-dose use
Administration: Remain upright ≥30 min; separate from calcium/iron; monitor renal function -
Analytical & Detection Methods
HPLC-CAD (mixed-mode column): 8-min stability-indicating assay for release & stability testing
Fluorescent “turn-off–on” sensor: N,S-doped carbon dots (microwave 60 s) detect Cu²⁺ or etidronate (linear range 30–210 µM)

Industrial Synthesis
Reaction of phosphorus trichloride with acetic acid → etidronic acid → controlled neutralisation with NaOH → Etidronate disodium (pharmacopoeia-grade).
